What is a Transition Program?
If you or a loved one have recently been admitted to a hospital or rehab due to an illness, fall, surgery, or a recent injury and the recovery time is uncertain on when and how well you will be able to care for yourself, we at Brookfield Assisted Living and Memory Care want to provide you the ability to rebuild your confidence in being able to take care of yourself.
The philosophy of assisted living emphasizes personal autonomy and privacy. Our facilities emphasize independence, homelike settings, and the ability to age in place. Our communities are designated as Level I or Level II Assisted Living.
